| Time: 8:00 am shotgun | Cost: Entry fee $70 per person. Includes cart, continental breakfast, lunch and door prizes. 10 point spread between partners. Maximum handicap of 36. |

| Format: 4 Flights. Send Name, Phone #, Address, and 8-digit UGA number, club name and club telephone number. Early postmarks or incomplete entry forms will not be accepted. The first 72 completed team entries will be accepted. |
